The affidavit is the last piece of paperwork in your KMU admission, and the one that confuses students most. Is it one affidavit or two? Can both go on one stamp paper? Does your college make it for you, or do you bring your own? Getting this wrong usually means a second trip to the stamp vendor and to your college.
This guide clears it up. It explains the two undertakings every MBBS and BDS student in Khyber Pakhtunkhwa's public colleges submits, what each one says, how to prepare them, and which colleges make their own. You can download both forms at the end of the guide.
The short answer: there are two affidavits, not one: the student undertaking and the father or guardian undertaking. Print each on its own stamp paper, never both on one sheet. Some colleges, such as Ayub Medical College (AMC) and Saidu Medical College (SMC), give you their own affidavits at admission, so do not prepare them in advance. Others, such as Bacha Khan Medical College (BKMC) and its dental section, expect you to bring them ready.
Why there are two affidavits
KMU's admission policy asks for undertakings from both the student and the parent or guardian at the time of admission. Students call them affidavits because they are printed on stamp paper and signed like a legal declaration. They are separate documents with separate signatures:
- The student undertaking is your own promise to follow the college's rules.
- The father or guardian undertaking is your parent's promise that you will keep yours, with a money penalty if you do not.
The student undertaking: what you are agreeing to
The student undertaking is a long list of promises. Read it fully before you sign. In plain words, you agree that:
- You have read KMU's admission policy and the college prospectus, and you will follow the college's rules.
- The information and documents you gave are correct. If anything is found false at any stage of the course, your name is struck off and legal action can follow, with no fee refund. You would also pay liquidated damages of Rs 100,000 for each year of study.
- You will keep at least 75% attendance and will not be promoted until you clear all subjects.
- You will wear the college uniform and take part in games and co-curricular activities.
- You will not demand hostel accommodation if none is available.
- You will stay out of politics of any type, and will not join any political party, organisation or student federation, or hold gatherings and processions on campus.
- You will settle disputes peacefully, never through violence or pressure.
- You will not bring alcohol, drugs or weapons to campus, damage property, sublet a hostel room or do anything that brings a bad name to the college.
- If you are expelled for such acts, you pay Rs 200,000 for each year of study. If you do not honour the college's decision, you are liable for Rs 400,000 per year to the Government of Khyber Pakhtunkhwa.
- Your admission stands cancelled if you were not entitled to it under the policy.
- You do not hold a double domicile.
At the end, you sign it and write your full name, father's name, CNIC number, permanent address and the date.
The father or guardian undertaking
This is the shorter form, printed in KMU's admission policy under the heading "To be filled by father/guardian of the candidate (to be submitted to the concern college)". In it, your father, mother or guardian:
- fully endorses the undertaking you gave and assures that you will abide by it during your stay at college;
- accepts liability to pay Rs 1,00,000 (one hundred thousand) for each year of study if you break that undertaking;
- assures that you will not take part in politics of any type, will not be a member of any political party, organisation or student federation, and will not attend any meeting that could lead to expulsion, with the principal's decision final.
The parent or guardian signs it as the deponent. Two witnesses sign it too, each writing their full name, father's name, CNIC number and present and permanent addresses. It is then attested with the signature and stamp of a Deputy Commissioner or First Class Magistrate.
How to prepare both affidavits
- Check your college first. If your college makes its own affidavits (see the next section), stop here and wait for its forms.
- Download both forms from the links at the end of this guide.
- Print each one on its own stamp paper. The student undertaking goes on one stamp paper; the guardian undertaking goes on another.
- Fill in the details in clear handwriting: names, CNIC numbers and addresses. Write the date on which you sign.
- Sign. You sign the student undertaking. Your father or guardian signs the guardian undertaking, with two witnesses who bring their CNICs.
- Get it attested. The guardian form needs the signature and stamp of a Deputy Commissioner or First Class Magistrate. If your college asks for attestation of the student form too, follow its notice.
Plain paper or stamp paper? The heading of the student undertaking in KMU's policy mentions plain paper, but colleges ask for both undertakings on stamp paper in practice. Follow your college's notice; if it says nothing, use stamp paper.
Can both go on one stamp paper?
Every year some students ask whether they can print one undertaking on the front of a stamp paper and the other on the back to save money. Please do not. They are two separate legal documents with different people signing them, and a college can refuse a sheet that combines them. Print each on its own stamp paper. You are joining a medical college; this is not the place to cut corners.
Does your college make the affidavit for you?
Colleges handle this differently, and it changes what you should do before reporting.
| College | Who prepares the affidavit | What you should do |
|---|---|---|
| Ayub Medical College (AMC), Saidu Medical College (SMC) and one or two others | The college prepares its own and does not accept outside copies | Do not make them in advance. Get them from the college at admission. |
| Bacha Khan Medical College (BKMC) and its dental section | The student | Prepare both affidavits yourself and bring them when you report. |
| Any other college | Varies | Check the college's admission notice or call its admission office before you buy stamp paper. |
This list is based on what our students found last year. Colleges can change their practice, so always confirm with your own college.
