Most UHS applications that run into trouble do so at the upload step, not the merit step. A missing side of a CNIC, a blurred result card or a file that is too large can get an application rejected without notice. This guide explains, from the UHS MBBS/BDS Prospectus for Session 2026-27, exactly which documents to upload for public sector MBBS and BDS admissions in Punjab, how to scan them and what to do in the situations that confuse students most.
The short answer: upload your Punjab domicile, Matric (SSC) and FSc (HSSC) result cards verified by your board, your CNIC or B-Form, a parent’s or guardian’s CNIC, a recent photograph and your signature with left thumbprint. Every file must be JPG or PNG and no larger than 500 KB, clear and readable, with each side of a two-sided document uploaded separately. IBCC attestation is not required at the time of applying for HSSC-pass students; it is needed after you join a college.

Candidates with A-levels, IB or another foreign qualification upload an IBCC equivalence certificate or provisional equivalence letter for both 10th and 12th grade, attested by IBCC. Quota seats need extra documents, covered below.
File format, size and scan rules
- JPG or PNG only, with a maximum of 500 KB per file. (This limit is new for 2026-27; earlier sessions allowed 1 MB, so do not follow old guides.)
- Scan the real document. The prospectus says only scanned copies of original documents should be uploaded, and that blurred or unclear documents will be rejected. A scanning app such as CamScanner is allowed.
- Two sides, two files. For double-sided documents such as the CNIC, upload a separate file for each side.
- Name files clearly, for example fsc-result-card.jpg, and open each one to check it before uploading.
- Translations. A document in a language other than English or Urdu needs an official notarized or certified English translation verified by the Pakistan Embassy or High Commission.
How to keep a clear scan under 500 KB
- Scan in good daylight on a flat, dark surface and crop away the background.
- In the scanning app, export as JPG at medium quality instead of “high” or “original”.
- Check the file size in your phone’s file manager. If it is still over 500 KB, reduce the quality one step or the page size slightly, then zoom in to make sure every number and stamp is still sharp.
- Never compress so far that marks, roll numbers or the board’s stamp become hard to read. A small file that cannot be read is worse than no file.
Result card or certificate: which one should you upload?
The prospectus asks for your SSC and HSSC result cards verified by the relevant board. Students often have different documents verified, so here is how we suggest handling each case:
- Only your result cards are verified: upload the verified result cards.
- Only your certificates are verified: upload the verified certificates.
- Both are verified, or a mix (for example a verified Matric certificate and a verified FSc result card): upload the verified version of each.
- The upload box uses a different name from the document you have (for example it says “certificate” and your verified document is a result card): upload your board-verified document in that box. What matters is that the document is verified and readable.
Whatever you upload, it must show the board’s verification stamp clearly.
Blank back sides, IBCC attestation and board receipts
The back of my document is blank. Do I upload it?
The prospectus asks for each side of a double-sided document as a separate file. If the back of a result card or certificate is completely blank, there is nothing to show, so the front is enough. If the back has a stamp, signature or verification mark, upload it as its own file.
My documents are attested by IBCC. Can I upload those?
Yes. IBCC attestation is not required at the time of applying for HSSC-pass students, but an IBCC-attested copy is still a verified copy of your document, so you can upload it. You will need IBCC attestation anyway after you join a college. For IBCC equivalence certificates (foreign qualifications), the prospectus requires IBCC attestation.
My board kept my documents for verification and gave me a receipt
Some boards keep documents during verification and give a receipt or token instead. The prospectus does not describe this situation, so here is a practical approach: scan your original document (or a clear photocopy) with the board’s receipt placed on it so both are visible, front and back, and upload those scans in the matching boxes. Keep the receipt safe and collect the verified documents as soon as the board returns them, because you will need originals and attested copies when you join a college. If you are unsure, check the UHS website and portal for any notice on this.
The board gave me a single verification page
Upload that page with your document in the same way, so the verification and your marks are both visible and readable.
Photo, signature and identity documents
- Photograph: colour, white background, taken within the last month. Female candidates may wear a dupatta or headscarf with the face uncovered. No caps, hats or sunglasses, and no editing or touch-ups.
- Signature and thumbprint: sign on white paper in blue ink, in your normal running hand, add your left thumbprint, and upload a clear photo of the page.
- CNIC or B-Form: front and back as separate files. A parent’s or guardian’s identity document is also required.
- Overseas Pakistanis and dual nationals on their reserved seats also upload an Iqama, residence permit, green card, Maple card or foreign passport.
Extra documents for quota seats
These are needed only for the matching reserved seats, not for open merit:
- Under-developed districts: Education Verification Certificate (template on the UHS website) covering class 1 to 12, verified by the district authorities and attested by the Deputy Commissioner, plus school leaving, character or provisional certificates.
- Cholistan: certificate issued by the Cholistan Development Authority (template on the UHS website).
- Students with disabilities: disability certificate from NCRDP or the Social Welfare Department; candidates then appear before a medical board.
Before you press submit
- Download and print your application, then check every entry and every uploaded file.
- If a file is wrong or unclear, replace it before final submission. You can edit until the deadline, and only your latest submission counts.
- After the deadline nothing can be changed, and the order of college preferences is locked.
- Submitting more than one application, for example with a different CNIC or email, leads to disqualification.

Frequently asked questions
What is the maximum file size for UHS MBBS documents in 2026-27?
500 KB per document, in JPG or PNG format, according to the UHS prospectus for Session 2026-27.
Can I upload PDF files on the UHS portal?
No. The 2026-27 prospectus allows JPG or PNG files only.
Is IBCC attestation required to apply for MBBS through UHS?
Not for HSSC-pass candidates at the time of applying; board-verified result cards are required instead. You will need IBCC attestation of your educational documents after joining a medical or dental college. Candidates with foreign qualifications need an IBCC equivalence certificate attested by IBCC.
Should I upload my result card or my certificate?
The prospectus asks for board-verified result cards. If your board verified your certificates instead, upload the verified certificates. Upload whichever version of each document is verified, as long as it is clear.
Do I need to upload both sides of my CNIC?
Yes. UHS asks for a separate file for each side of a double-sided document such as the CNIC.
Can I use CamScanner for my documents?
Yes. The prospectus names apps such as CamScanner for preparing scans. Make sure each file stays under 500 KB and is fully readable.
What happens if I upload a blurred or wrong document?
UHS can reject the application without notice. Replace any unclear file before the deadline; after the deadline no changes are allowed.
Do I send hard copies of my documents to UHS?
No. You apply and upload online. You will deposit your original documents, with attested copies, at the college you are selected for when you join.
